I have at least 2 years experience caring for children

I declare that I am both physically and mentally fit to work with children

I have an enhanced CRB which is less than 3 years old
    (FYI if you do not have a CRB we cannot process your application until you obtain one.
    You can apply for a CRB through www.work-zone.org.uk)
